"Preexposure prophylaxis (PrEP) in men who have sex with men (MSM) in Bouaké, Côte d’Ivoire: A qualitative evaluation of acceptability and feasibility" The Open AIDS Journal  (2019) 13:49. 

Résumé: L'article présente les résultats d'une étude qualitative qui visait à évaluer l'acceptabilité et la faisabilité de la prophylaxie préexposition (PrEP) chez les HARSAH de Côte d'Ivoire via des groupes de discussion et des entrevues individuelles. L'analyse repose sur un cadre théorique, le cadre des domaines théoriques, afin de bien cerner les déterminants de l'acceptabilité de la PrEP dans cette population.

Abstract : 

Background: HIV remains an important public health issue throughout the world. In Côte d’Ivoire, a high burden of HIV is observed in men who have sex with men (MSM). Objective: We assessed the acceptability of Pre-Exposure Prophylaxis (PrEP) among men who have sex with men (MSM) in Bouaké, Côte d’Ivoire. Methods: We conducted and analysed four focus groups with 31 HIV-negative MSM and eight in-depth individual interviews with participants recruited from the focus groups. Results: Four MSM (13%) were aware of PrEP before participating in the study. All the participants were interested in taking PrEP if available: 19 (61.3%) would prefer the daily regimen and 12 (38.7%) would opt for the on-demand regimen. Many advantages of PrEP were mentioned: protection incase of a condom break, protection in case of high-risk sexual behaviour, self-reliance, decreasing HIV fear and ease of use. Barriers to the use ofPrEP included: it does not protect against other Sexually Transmitted Tnfections (STIs), taking a pill regularly is necessary, the size of the pill, possibility of side effects, the cost and accessibility. Six participants (19.3%) admitted that they would use condoms less if they take PrEP. Conclusion: Findings indicate that PrEP is acceptable within the MSM community. Implementation should be done rapidly, and PrEP should be part of a global prevention program which includes counselling, STI screening and promotion of safe sex practices. Health authorities should consider PrEP for all high-risk groups to avoid worsening stigmatization by targeting MSM only.

"Development of a Method to Locate Deserts and Food Swamps Following the Experience of a Region in Quebec, Canada - [Développement d’une méthode pour localiser les déserts et les marais alimentaires suite à l’expérience de la région de Lanaudière, Québec, Canada]". 

Résumé : La saine alimentation est un déterminant important de la santé. L’adoption d’une saine alimentation a longtemps été considérée comme étant tributaire de facteurs individuels seulement. À ceux-ci, il appert que des facteurs collectifs et environnementaux, incluant des politiques publiques, influencent également les choix alimentaires en les rendant plus faciles à faire. En lien avec une démarche de sensibilisation sur les caractéristiques de l’environnement alimentaire d’une région socio-sanitaire, l’objectif est d’illustrer le développement de nouveaux indicateurs permettant de localiser les secteurs qualifiés de « déserts alimentaires » et de « marais alimentaires ». À l’aide d’une méthode basée sur les systèmes d’information géographique (SIG), il a été possible d’opérationnaliser des indicateurs de marais et de déserts alimentaires pour la région à l’étude. Les résultats révèlent que 8 % de la population de la région habite des secteurs qualifiés de déserts alimentaires et 3 % des secteurs qualifiés de marais. La région de Lanaudière est déjà activement engagée dans la modification des environnements pour les rendre favorables à la saine alimentation pour tous. La cartographie des déserts et des marais alimentaires sera utilisée pour accompagner les réflexions avec leurs partenaires. Elle permettra de sensibiliser et de mobiliser les partenaires au développement d’une stratégie globale pour améliorer l’environnement alimentaire.

Abstract : Unhealthy eating and food insecurity are recognized risk factors for chronic diseases. Collective and environmental factors, such as geographic access to food condition food choices. The objective of this study was to map food deserts and food swamps in Gaspesie, a region of Quebec (Canada), using geographical information systems (GIS) and field validation. Eleven sectors (5 rural and 6 urban) where 5% of the Gaspesie population lives were considered food deserts. Eight sectors (all rural) constituting 4.5% of the population were considered food swamps. Nearly 88% (3/8) of food swamps were located in disadvantaged and very disadvantaged areas. The Gaspesie region is already actively involved in changing environments to make them conducive to healthy eating for all. The mapping of food deserts can support intersectoral collaboration on food security. Food swamp mapping will make it possible to more accurately characterize the existing food environment in the region. Both indicators will be useful in raising awareness and mobilizing partners for a comprehensive strategy to improve the food environment that is not only based on the food desert indicator alone but also takes into account the presence of food swamps.
